Our restorative team helps residents maintain their current level of function after being discharged from therapy. This care includes walking, ROM, exercises, and stretches. They also work with residents that need assistance with meals. While performing this care with the resident, should they recognize any changes, the CNAs communicate those changes/declines to the restorative nurse.
The restorative nurse works closely with therapy to identify residents' declines and needs through routine assessments. She refers residents with declines to therapy. She also ensures that the FMP (functional maintenance plan) for the residents is carried out.
